After more than fifteen years practicing emergency medicine—from critical access hospitals to busy community emergency departments—I came to appreciate that experienced physician schedulers already know what makes a healthier schedule. Forward progression, minimizing quick turnarounds, respecting circadian rhythms, and balancing workload aren't new ideas—they're principles that seasoned schedulers intuitively apply every day. The real challenge isn't knowing what makes a good schedule—it's translating that human expertise into something a computer can objectively understand, measure, and apply consistently while balancing staffing requirements, physician preferences, distribution, and operational constraints.

Working as both a physician and a scheduler also gave me a unique perspective on the limitations of traditional workforce management software. Building a schedule often requires navigating hundreds of configuration settings, competing rules, and manual adjustments to produce an acceptable result. Yet despite all of that sophistication, most scheduling platforms are designed to optimize staffing—not the people being staffed. They rarely account for human physiology, circadian alignment, fatigue mitigation, or the cumulative effects that schedule design has on clinician wellbeing.

AIER was built to bridge that gap. Rather than replacing the judgment of experienced schedulers, it captures the principles they already use and combines them with clinical informatics, evidence-based scheduling science, and the Circadian Alignment Index (CAI) to make healthier scheduling objective, measurable, and scalable.

Technology should support clinical judgment—not replace it. My goal is simple: reduce administrative load, improve transparency, and help emergency departments build schedules that are not only operationally sound, but healthier and more sustainable for the clinicians who provide care every day.
